  • [35A-03-4] Dead roaches on premises. Observed one dead roach on floor under counter where bag in box storage is located. Person in charge discarded.
  • [21-05-5] Cloth used as a food-contact surface. Observed cloth in direct contact with bread in warmer in kitchen. Person in charge added barrier during inspection.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Soda plates at server area with mold like substance buildup. Also, gaskets of lowboy breaded reach in cooler with mold like substance accumulation. Also, can opener exterior with debris accumulated. Person in charge pulled to clean.
  • [01B-01-4] Dented/rusted cans present. See stop sale. Observed one can of fruit salad with dent along seam in walk in cooler.
  • [12A-29-4] Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee pick up shrimp shell from floor with gloved hands, then continued working with food without hand wash. Spoke to employee who removed gloves and washed hands.
  • [08A-20-5] Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. In double door reach in freezer uncooked chicken tenders in open packaged stored over uncovered container of raw shrimp. Person in charge moved to appropriate location.
  • [29-37-4] Spray hose at dish sink lower than flood rim of sink. Spray hose at sink near breading room pass through lower than flood plain, no vacuum breaker on hose. Person in charge raised.
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Beans baked in walk in cooler made yesterday stored above cooling hot items at 44F. Person in charge states cooling items have been in unit for short period of time. Moved beans to other location in walk in cooler.
  • [03F-10-5] No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Establishment holds salad, dressings, shrimp, on time, not indicated on time paper work. Person in charge updated during inspection.
  • [41-17-4] Spray bottle containing toxic substance not labeled. Several spray bottles with spic and span, chemicals per person in charge in breading room, at sink in kitchen unlabeled. Person in charge labeled appropriately.
