  • [14-11-5] Equipment in poor repair. Observed gasket on walk-in-cooler torn. Also, right side of lid at pizza make station in disrepair/ duct taped/ taped.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gasket and frame on walk-in-cooler door soiled with mold like substance.
  • [29-11-4] Water leaking from pipe and/or faucet/handle. Observed pipe at hand wash sink across from tripe sink leaking
  • [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed bag of employees personal belongings stored on top of single use pizza boxes in back storage room. Operator removed bag to properly store.
  • [53A-02-7] Manager or person in charge lacking proof of food manager certification and no other certified food service manager employed at this location. Observed operator on duty unable to provide proof of a certified food manager certification for herself or any other managers employed. Inspector checked Servsafe site for Marcio-General Manager but unable to find.
  • [36-27-5] Wall soiled with accumulated grease, food debris, and/or dust. Observed wall near walk-in-cooler, and near hand wash sink near pizza make station soiled. Also, multiple ceiling vents soiled/ dusty.
  • [25-06-4] Single-service articles not stored inverted or protected from contamination. Observed multiple folded pizza boxes behind POS station and next to glass door reach-in cooler with debris and dust like substance.
  • [53B-01-5] No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed operator unable to provide proof of employee food safety training during this inspection.
  • [22-43-4] Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Observed Triple Sink (Quaternary 0ppm). Operator tried running sanitizer at triple sink but unable to get a minimum of 150ppm. Operator located bleach in managers office and remade solution of chlorine sanitizer and retested at 50ppm.
  • [11-26-1] No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Operator unable to provide proof that employees are informed of reporting symptoms related to food borne illnesses to person in charge. Distributed health form to operator during this inspection and she read and signed form during this inspection.
