[10-01-5] In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. At salad bar flip top, observed tongs submerged in container of lettuce and water.
[12A-07-5] Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed operator taking off his gloves after washing dirty dishes. The same operator puts his bare hands on his mouth, to then don new single use gloves with no hand wash. Operator then switched tasks to prepare a Togo order and dice tomatoes
[21-11-4] Sanitizing solution for wiping cloths not free of food debris and visible soil. Observed sanitizer buckets with rags inside, filled with murky grey water and food debris, in triple sink area
