- [11-27-4] Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Provided to operator via email.
- [13-07-4] Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed female employee wearing a watch and bracelet while in food preparation.
- [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed soiled exterior of reach in cooler in the kitchen next to prep tables. Observed rust on hood of vent above 3CS. Also observed soiled door gaskets for walk in cooler and freezer, and multiple reach in coolers in the establishment. Observed hood system above 3CS rusted.

- [05-08-4] No probe thermometer provided to measure temperature of food products when asked. Observed one provided was in dis repair. Operator was able to provide a new one before inspection completed.

- [03D-02-5] Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. Observed cooked sweet potato (53F - Cooling) in the prep cooler. Per operator item was cooked this morning at 6am, still has not reached 41°F at 6 hour mark, temperature taken at 12:07pm. . A stop sale was issued. Operator discarded.
- [01B-02-5] Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed cooked sweet potato (53F - Cooling) in the prep cooler. Per operator item was cooked this morning at 6am, still has not reached 41°F at 6 hour mark, temperature taken at 12:07pm. Operator discarded. Also observed deep fried chicharron (120F - Hot Holding)in the hot holding in front counter, per operator it was placed in hot holding unit around 6am. Also observed butter (80F - left on counter for panini press since 6am).
- [36-02-5] Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Observed in the kitchen area.
- [36-68-5] Hood ventilation system inadequate as evidenced by grease accumulation on hood and on sprinkler spouts.
- [21-10-4] Soiled dry wiping cloth in use. Observe on prep counters in the kitchen. Manager replace with clean clothes for use.
