  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On ice at cookline: egg wash (54F - Cold Holding). Manager stated the liquid egg wash was placed on ice approximately one hour prior and added water to the ice to aid in cooling the wash to 41F.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Area around nozzles of fountain machine soiled with mold-like substance.
  • [24-08-4] Equipment and utensils not properly air-dried - wet nesting. Wet nested plastic pans on clean dish rack in warewash area.
  • [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Several employee jackets stored on top of canned food shelf in dry storage area. Manager removed the jackets from the area during this inspection.
  • [41-07-4] Container of medicine improperly stored. Bottle containing "tylenol" stored on shelf beside canned vegetables and above bins of breading in dry storage area. Manager removed the medicine from the shelf during this inspection.
  • [14-01-5] Bowl or other container with no handle used to dispense food. Single-use cup without handle stored down inside corn meal in dry storage area. Manager removed the cup during this inspection.
  • [31A-09-4] Handwash sink not accessible for employee use at all times. Handwash sink at fountain machine blocked by dump bucket and trash can. Manager unblocked the sink during this inspection.
  • [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Backflow prevention device missing at spigots on front and back of building.
  • [02C-03-5] Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Open jug of milk missing date mark inside walk-in cooler. Manager stated the milk was opened on 5/1 and added the proper date mark during this inspection.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Interior of ice machine door soiled with brown substance.
  • [16-46-4] Old labels stuck to food containers after cleaning. Remnants of old labels stuck to four pans on clean dish rack.
  • [01B-28-5] Date marked combined ready-to-eat, time/temperature control for safety food not consumed/sold within 7 days of preparing/opening the earliest ready-to-eat time/temperature for safety ingredient. See stop sale. Large container of sliced tomatoes with date mark of 4/22 inside walk-in cooler.
  • [16-32-5] Incorrect chemical test kit provided for measuring the concentration of the sanitizer solution used in the three-compartment sink and/or dishmachine. Manager provided quaternary test strips for chlorine dish machine.
  • [12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Several employee drinks on prep table in kitchen above tea bags. Manager removed all drinks from the table during this inspection.
  • [08B-49-4] Employee personal food not properly identified and segregated from food to be served to the public. Personal food stored above condiments inside reach-in cooler. Manager removed the personal food from the cooler during this inspection.
  • [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. Pan containing raw pork stored on shelf above wrapped potatoes inside walk-in cooler. Manager moved the raw pork to a lower shelf during this inspection.
  • [22-41-4] Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dish machine chlorine 0ppm. Manager called Ecolab arrived and repaired the machine during this inspection, then chlorine 50ppm.
