  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed cooked sausage (47F-51F - Cold Holding) in reach-in cooler next to kitchen entry. Operator stated employee may have panned it up this morning. Operator stated he will keep pan in cooler to chill sausage.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets on upright reach-in glass door cooler soiled. Also, exterior of mop bucket, multiple sheet pans, exterior of bucket with salt and sugar in prep area soiled.
  • [35A-02-7] Live, small flying insects found Observed one flying insect near front counter.
  • [29-11-4] Water leaking from pipe and/or faucet/handle. Observed faucet at mop sink leaking.
  • [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand wash sink behind pizza oven and also no paper towels in unisex restroom. Operator replenished paper towels at both hand wash sinks.
  • [14-31-5] Nonfood-grade bags used in direct contact with food. Observed sausage stored in direct contact in non food grade thank you bag in reach in deep freezer.
  • [36-73-4] Floors/ Walls/ Ceiling soiled/has accumulation of debris. Observed walls and ceiling in kitchen area soiled.
  • [53A-02-7] Manager or person in charge lacking proof of food manager certification and no other certified food service manager employed at this location.. Observed CFM certification expired in Feb 2026 and no other CFM at this establishment.
  • [41-10-4] Toxic substance/chemical improperly stored. Observed spray bottle containing toxic substance stored next to single serve crush pepper packets. Operator removed spray bottle of deodorizer to properly store.
  • [22-08-4] Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed interior of microwave on cook line soiled
  • [09-20-5] Employee touching ready-to-eat food with their bare hands - ready-to-eat food intended to be heated as a sole ingredient not immediately heated to 145 degrees F after bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee handling ready to eat calzones that will not be heated immediately to a minimum of 145F. Discussed with employee and he washed hands and put on gloves to finish task with calzones.
