- [12A-10-4] Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee at cookline scratched face with bare hand, then put on gloves and handled utensils at the cookline. Manager had the employee stop, wash hands, and put on new gloves before continuing to work at the cookline.

Manager had an employee rearrange the eggs during this inspection.

The manager stated the melon was placed on the counter in ice this morning from another cooler. The pan of melon is sitting in another pan of ice. The manager added water to the ice to increase the cooling rate of the cantaloupe. cut melon (47F 10:35, 41F 11:30 - Cooling)
- [14-01-5] Bowl or other container with no handle used to dispense food. Cup without handle stored down inside pan of food inside reach-in freezer near cookline. Manager removed the cup from the pan during this inspection.
- [35A-03-4] Dead roaches on premises. Two dead roaches on floor in back of kitchen, near back door. Manager swept the area during this inspection.
- [36-51-4] Building components, attachments or fixtures in poor repair. 1. Two small holes in wall below three-compartment sink. 2. One Absorbent and porous acoustic ceiling tile in-use in center of kitchen.
