  • [35A-02-8] Live, small flying insects found. Observed 1 flying insect near dish machine.

  • [22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed interior of ice machine soiled with mold like substance near ice chute of unit.
  • [09-01-4] Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed in sushi area employee using bare hands to handle ready to eat krab sticks. Manager intervened and spoke to employee. Employee washed hands and placed gloves on.
