  • [29-09-4] Faucet/handle missing at plumbing fixture. Cold water faucet handle missing at handwash sink near cookline.
  • [32-05-4] Bathroom facility in disrepair. Toilet seat missing from toiled in mens restroom. The mens restroom is labeled as "Closed" with a sign on the door and the womens restroom is being used as a unisex restroom for customers.
  • [36-30-4] Ceiling and/or wall not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom. Walls and ceiling in food storage area in back of kitchen coated in rough spray foam.
  • [35A-01-4] Nonservice animals in the food establishment or on premises. Small white dog sitting in lap of customer while eating in dining area.
  • [01C-03-4] Clam/mussel/oyster tags not marked with last date served. One oyster tag with sell-by date of 06/95/2025 missing date last oyster was sold. Inspector reviewed the process with the cook during this inspection.
  • [33-34-4] Storage area not maintained clean and organized. Mop sink is inaccessible. Maintenance equipment stored beside and on top of mops sink, blocking area.
  • [35A-03-4] Dead roaches on premises. Four dead roaches on floor inside walk-in cooler. An employee swept the area during this inspection.
  • [02A-02-5] Raw or undercooked oysters offered and consumer advisory provided does not contain the required verbiage. No raw oyster consumer advisory posted on wall or on menu. Raw oysters are offered on menu. Establishment only has the standard consumer advisory not for oysters. Manager posted the raw oyster consumer advisory on wall during this inspection.
  • [24-08-4] Equipment and utensils not properly air-dried - wet nesting. Wet nested drink cups on counter behind bar.
  • [48-04-4] Propane tank (larger than 2.7 lb. water capacity/1 lb. gas capacity) located inside of the building. For reporting purposes only. Two propane tanks stored inside back of kitchen, near walk-in cooler.
  • [33-15-4] Garbage can located outside has no lid or lid open/broken. Outdoor trash can in outdoor seating area has no lid.
  • [36-51-4] Building components, attachments or fixtures in poor repair. 1. Several potholes in concrete kitchen floor, near cookline and mop sink areas. 2. Several missing floor tiles in bar area. 3. Door release button broken on inside of walk-in cooler door.
  • [35B-05-4] Outer openings not protected during operation and vermin and/or environmental cross contamination present. Gap below blue door leading outside from kitchen. Four dead roaches inside walk-in cooler near this back door.
  • [36-50-4] Unclean building components, attachments or fixtures. 1. Walls and pipes on walls throughout kitchen soiled with dirt and/or dust. 2. Floor at cookline heavily soiled with grease and/or food debris. 3. Floor iside walk-in cooler soiled with dirt and/or debris. 4. Floors along perimeter of bar area soiled with dirt and/or debris.
  • [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. Package of raw steak stored on shelf directly above wrapped container of sliced fully-cooked bologna inside walk-in cooler. An employee moved the raw steaks to the bottom shelf during this inspection.
  • [24-05-4] Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Metal bowl not inverted on shelf below wall A/C unit in kitchen. An employee inverted the bowl during this inspection.
  • [14-38-4] Food storage container/container lid cracked or broken. Several cracked and/or chipped plastic pans on clean dish rack near cookline.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. 1. Wire shelving near cookline heavily soiled with grease. 2. Hood filters above cookline soiled with grease.
