  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Three uncooked cheese pizza stored on kitchen prep table next to auto fry machine tempted at 68°F to 70°F at 12:13 PM, left out on counter at 10:10 AM.
  • [25-05-4] Single-service articles improperly stored. Two boxes of single service cup lids stored on floor next to three compartment sink.
  • [51-11-4] Carbon dioxide/helium tanks not adequately secured. Four unsecured tanks next to bag in box, soda syrups and ice machine room.
  • [31B-04-4] No handwashing sign provided at a hand sink used by food employees. Located next to autofry machine.
  • [08B-38-4] Food stored on floor. Box of Frito lay chips, stored on floor next to three compartment sink. One box of soda syrup, and one case of bottled water store on floor next to ice machine.
  • [22-20-5] Accumulation of black mold-like substance in the interior of the ice machine.
  • [36-27-5] Wall soiled with accumulated grease, food debris, and/or dust. Black mold-like substance on bottom of wall, located between wall and ice machine.
  • [53B-02-5] No proof of required state approved employee training provided for employee hired more than 60 days ago. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Employee, Brooklyn, hired for over 1 year without food handler training.
  • [50-09-4] Current Hotel and Restaurant license not displayed. Manager posted license at time of inspection.
  • [36-02-5] Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. On floor of combo reach in cooler/reach in freezer, next to bag in box, soda syrups.
  • [53A-07-6] No certified food manager for establishment. A list of accredited food manager certification examination providers can be found at http://www.myfloridalicense.com/DBPR/hotels-restaurants/food-lodging/food-manager/ Previous CFM left, no manager available.
  • [41-17-4] Spray bottle containing toxic substance not labeled. 4 unlabeled chemical spray bottles stored on speed cart and on cleaning storage rack inside mop sink room.
