- [36-37-5] Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Absorbent and porous acoustic ceiling tiles in-use throughout kitchen.
- [08B-38-4] Food stored on floor. Box containing fountain soda syrup stored directly on floor near back door of kitchen. Manager placed the item on a shelf during this inspection.
- [33-19-4] Garbage on the ground and/or pad around dumpster. Trash scattered inside dumpster enclosure and along bushes at left side of building.
- [35B-02-4] Insect control device installed over food preparation area. Insect pest control device installed on wall directly above prep table in kitchen.

- [12B-13-4] Opened employee beverage container in a cold holding unit with food to be served to customers. Employee "gatorade" drink bottle stored on shelf above wraps inside walk-in freezer. Manager removed the bottle from the shelf during this inspection.

Pliers and two screwdrivers stored on magnetic strip on wall directly above prep sink. Manager removed the tools from the area during this inspection.
- [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Inside refrigerated make-table at front counter: roast beef (51F - Cold Holding); ham (47F - Cold Holding); turkey (47F - Cold Holding). Manager stated these items sat out at room temperature for approximately 20 minutes one hour prior, while he was doing inventory. Inspector had manager move the items into the walk-in freezer to rapidly cool to 41F. roast beef (51F 9:40, 40F 10:15 - Cooling); ham (47F 9:40, 38F 10:15 - Cooling); turkey (47F 9:40, 40F 10:15 - Cooling)
- [05-05-4] Ambient air thermometer in holding unit not accurate within plus or minus 3 degrees Fahrenheit. Digital ambient thermometer outside of walk-in cooler reads 47F. Actual ambient air temperature is 38F.
