  • [14-09-4] Cutting board has cut marks and is no longer cleanable. Observed cutting boards with grooves throughout at make tables.
  • [22-08-4] Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed food splatter on inside of microwave.
  • [03A-05-5] Time/temperature control for safety food not maintained frozen solid in a freezer. Observed freezer temperature at 32F and rising. Observed employee put in work order for repair and keeping freezer door shut. Refrigerator company is on their way out for repairs within the hour.
  • [12A-16-4] Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed employee come into work station and start working without washing hands first.
  • [12A-07-5] Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee changing gloves without hand washing.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed on make lines, cutting boards with black mold like substance in cut grooves.
  • [46-01-4] Marked exit/path to marked exit blocked. For reporting purposes only. Observed a box blocking exit back door from delivery. Observed employee move out of the way un blocking path.
