[14-09-4] Cutting board in back prep area on top of table has cut marks and is no longer cleanable.
[12B-02-4] Employee eating in a food preparation or other restricted area. Observed employee eating on prep table next to other employee preparing noodles. Manager had him move to the dining room.
[52-01-4] Identity of food or food product misrepresented. Escolar represented as "white tuna" on menu. Discussed with manager and he had an employee tape over the "white tuna" on all menus during inspection.
[08B-38-4] Food stored on floor in walk in freezer. Employees had no room to move items off floor. Discussed better organization with manager so no food is on the floor.
[14-86-1] Non-food grade paper/paper towel used as liner for food container. Observed salmon wrapped in paper towel in sushi bar. Employee removed and re-wrapped with clear wrap.

Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed the dish machine testing at 0ppm. Discussed setting up three compartment sink until dishwasher machine is sanitizing correctly. Texted three compartment sink at 50ppm.
