- [01B-01-4] Dented/rusted cans present. See stop sale. Dry storage racks in middle of back kitchen 2 cans pinto beans 2 cans black beans heavily dented.
- [16-62-1] No measuring device available for measuring utensil surface temperature when using hot water as sanitizer in a dishmachine.
- [29-08-4] Plumbing system in disrepair. Under drink station in dining room, pipes leaking left side.
- [33-06-4] Trash receptacles not provided where needed in establishment. In kitchen soup area, no readily available trash can near handwash sink. Operator moved small trash can to sink.
- [46-01-4] Marked exit/path to marked exit blocked. For reporting purposes only. Front marked door one side locked, back hallway of kitchen blocked with cart, operator removed.

Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired. Dishwasher (Temperature 150F) after running multiple times, operator set up chlorine backup for system at 25ppm, and resanitized dishware.
