  • [25-05-4] Single-service articles improperly stored. Box of to go bags on floor in front of dry storage rack. Manager moved box to shelf.
  • [35A-03-4] Dead roaches on premises. Observed one (1) dead roach on floor of lobby in front of register. Manager cleaned up and discarded.
  • [12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee drink cup observed on dry storage rack above to go bags. Manager discarded drink cup. Also, employee drink cup stored on bottom shelf of cook line next to fryer baskets. Manager discarded drink.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Interior of hot water nozzle soiled with white build up.
