- [14-11-5] Equipment in poor repair. Observed cracked container holding chips in the hot holding station parallel to the drive-through window.
- [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed build up on gaskets in reach in hot holding unit next to cook line. Build up observed underneath ice shoot at the drive through soda machine. Observed build up on exterior of soda bibb hoses.

- [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Brass valve attached to orange hose in hose at mop sink. Vacuum breaker present, but was not attached. Manager stated that the vacuum breaker did not fit the hose. Suggested to change out the hose to manager.
- [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed build up in reach-in hot holding unit next to fryer near cook line. Observed build up in right side hot water tap in interior of the nozzle. Food build up in main make line reach in cooler on side with hand wash sink.
- [33-19-4] Heavy leaf debris build up in enclosed dumpster area.
- [03B-01-6] Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Refried beans temped at 116f in the hot holding table and were prepared less than 30 minutes prior. Manager reheated to 167f.
- [29-49-6] Standing water in bottom of reach-in-cooler underneath main make line across from drive through window, parallel to soda machine.
