  • [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. Container of raw calamari stored over pan of cooked chicken in flip top cooler in kitchen. Operator moved calamari to bottom shelf.
  • [10-07-4] In-use utensil stored in standing water less than 135 degrees Fahrenheit. Clean utensil stored in container of standing water at 82 F on prep table at cook line. Employee discarded water.
  • [11-27-4] Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Emailed poster to operator.
  • [31B-04-4] No handwashing sign provided at a hand sink used by food employees. No handwashing signs in both bathrooms.
  • [41-15-5] Wiping cloth solution exceeding the maximum concentration allowed stored in a location that could result in the cross contamination of food, equipment, utensils, linens, single-service, or single-use articles. Chlorine wiping cloth sanitizer stored in bucket at over 200 ppm. Operator discarded sanitizer, to prepare correct sanitizer.
  • [55-01-5] Establishment charging operational charges without properly disclosing the information on the menu/web/app and the bill/receipt. Gratuity of 20 % of 4 or more listed on receipt, not listed on menu.
  • [03D-15-4] Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. In flip top cooler, Raw Fish (48F - Ambient Cooling) at 1 :45 pm, since 11:30 am to 51 F at 2:30 pm. Raw Shrimp (50F - Ambient Cooling) to 50 F at 2:30 pm. At this rate , proper cooling will not occur. Food was placed in ice to reach proper temperatures.
  • [22-12-5] Metal stem-type thermometer soiled. Employee cleaned thermometer.
  • [01B-02-5] Stop Sale issued on time/temperature control for safety food due to temperature abuse. Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. In flip top cooler, Cooked Chicken (50F - Heated Cooling). Per operator, food prepared 2 days ago and held in cooler. Food was discarded. See Stop Sale.
  • [14-01-5] Bowl or other container with no handle used to dispense food. Plastic cup without handle stored in bucket of Parmesan cheese on prep table. Employee discarded cup.
