  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Cutting board soiled with mold-like substances.
  • [31A-09-4] Handwash sink not accessible for employee use at all times. Back handwashing sink blocked by two rolling bins. Manager moved out of the way.
