- [29-03-4] Water draining onto floor surface. Water draining on the floor under the dishroom table. Operator states the faucet broke and will be repaired today.
- [36-17-5] Floor tiles missing and/or in disrepair. Multiple floor tiles missing and/or chipped in dishroom and at back exit door. Operator states repair materials are on site and will be repaired next week.
- [29-49-6] Standing water in bottom of reach-in-cooler. Standing water observed in bottom of small reach-in cooler at end of the make line and in expo line reach-in cooler.
- [23-24-4] Buildup of food debris/soil residue on equipment door handles. Black and pink substance observed on lid handle and rim along the handle on both doors of the ice machine. Employee cleaned both lids during inspection.
- [29-42-4] Vacuum breaker missing/incorrectly installed at mop sink faucet or on fitting/splitter added to mop sink faucet. On mop sink with added double splitters, vacuum breaker missing from non-chemical side with attached red hose. Operator has repair person called to complete installation.
- [14-69-4] Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup up observed in the walk-in freezer.
