  • [21-04-4] In-use wet wiping cloth/towel used under cutting board. Observed wet wiping cloth under cutting board on prep table
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Hood vents are build up with grease
  • [29-49-6] Standing water in bottom of reach-in-cooler. Observed at make station flip top
  • [31A-11-4] Handwash sink used for purposes other than handwashing. Observed both hand wash sink had pitcher stored in sink. Operator removed both items
  • [36-68-5] Hood ventilation system inadequate as evidenced by grease accumulation on walls/ceiling.
  • [22-16-4] Reach-in cooler interior/shelves have accumulation of soil residues. Make station flip top reach in cooler interior is soiled
  • [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. Observed in 2 door reach in freezer raw chicken over RTE items Reach in freezer is disarray
  • [32-04-4] Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Both bathrooms.
  • [25-06-4] Single-service articles not stored inverted or protected from contamination. Plastic forks,spoons, and to go trays not inverted or protected
  • [38-01-4] Light shield damaged/in disrepair. One light shield is damaged by entrance of kitchen
  • [08B-12-5] Stored food not covered. Observed In 3 door reach in cooler food is not cover for Cold Holding
  • [14-86-1] Non-food grade paper/paper towel used as liner for food container. Observed in 3 door reach in cooler non food grade plastic bag. Grocery bags use to store food items.
  • [36-17-5] Floor tiles missing and/or in disrepair. Observed floor tiles throughout the kitchen
  • [36-62-4] Light not functioning. One ceiling light by entrance to the kitchen is burn out
  • [14-01-5] Bowl or other container with no handle used to dispense food. Observed to go plastic bowls store in sugar container use has a scoop with no handle. Operator removed items
  • [14-33-4] Reach-in cooler shelves with rust that has pitted the surface. Observed bottom of make table
