[12A-02-4] Server handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing hands. Employee brought dirty dishes from dinner area into kitchen, then went out of kitchen and prepared drinks and handled clean plates without washing hands. Informed operator of proper handwashing, employee then washed hands.
[08A-13-5] Raw animal food stored in top portion of make table over ready-to-eat food in bottom portion of make table - no complete physical barrier between top and bottom. Raw tuna stored in top part of make table, carrots stored underneath, no barrier. Operator moved and stored properly.
[03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. deserts flip top; whipped cream (46F - Cold Holding) per operator out of temperature for approximately 1 hour. Operator reduced amount in container and put to quick chill. Not prepared or portioned today.
[31A-11-4] Handwash sink used for purposes other than handwashing. Hand washing sink used as dump sink at bar area. Informed operator who removed items from sink.
[29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Located at mop sink splitter, hose attached with no vacuum breaker.
