- [16-03-4] Accumulation of debris inside warewashing machine. Observed mold like substance inside the dishwasher located on the first floor.
- [41-17-4] Spray bottle containing toxic substance not labeled. Missing label for spray bottle with chemical product ( degreaser ) . Employee labeled the bottle.
- [03F-10-5] No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Observed missing TPHC procedure on place for Maki Son and Poke Son . Procedure was provided and filled it out.
- [02D-12-1] Juice packaged in the establishment that has not been specifically processed to prevent, reduce, or eliminate the presence of pathogens not bearing a warning label. Observed missing labels for fresh veggie juices that have been processed on site and packaged inside sealed plastic bottles in advance to customer orders. Juices have not been pasteurized. Person in charge discarded them . GM stated they will not be exposed and sold in the same way until they have the correct labels.

- [09-17-4] Employee using bare hand contact with nonready-to-eat food (such as raw animal food or flour) unnecessarily without the use of gloves, deli paper, scoops, tongs or other utensils. Observed owner of Shores venue touching the fish with bare hands unnecessarily. Owner was coached and explained about it. He washed his hands and placed gloves on.
- [12A-12-4] Employee switched from working with raw food to be touching clean containers and utensils without washing hands. Observed owner of Shores venue touching the fish with bare hands unnecessarily. Owner was coached and explained about it. He washed his hands and placed gloves on.
- [12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee open cup with coffee on prep table.
- [01C-05-4] Clam/mussel/oyster tags not maintained in chronological order according to the last date they were served in the establishment. Observed oyster tags not organized in chronological order. Observed at the Shores.
- [31A-09-4] Handwash sink not accessible for employee use at all times. Observed some metal containers blocking the access to the hand sink located at the ware washing area, first floor. Employee moved all of them immediately.
- [22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed accumulation of mold like substance inside the ice ,machine used for drinks.
