- [35A-03-4] Dead roaches on premises. 2 dead roaches, by the storage closet in the kitchen by the back door. 3 dead roaches, under the 3 compartment sink. The operator swept up the dead roaches and sanitized the area.
- [35B-01-4] Exterior door has a gap at the threshold that opens to the outside. The exit door is in the kitchen. You can see sunlight all around the door, and in some spots there are gaps.
