[51-11-4] Carbon dioxide tank near soda boxes not adequately secured. Operator secured with a chain.
[31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink in restroom in back of kitchen. Operator says they do not use this restroom but I explained all restrooms that are part of the business have the same requirements to stay stocked with soap, paper towels and a handwash sign. Operator restocked with paper towels.
[22-08-4] Interior of pizza oven has accumulation of black substance/grease/food debris.
[22-20-5] Black mold-like substance in the interior of the ice machine. Not observed in areas making contact with ice.
