  • [35A-03-4] Dead roaches on premises. 1 dead roach, waitress station. 2 dead roaches, in the dry storage area. 1 dead roach, in front of the walk in cooler. The operator swept up the dead roaches and sanitized the area.
  • [10-20-4] In-use tongs stored on equipment door handle between uses. The operator moved the tongs.
  • [22-08-4] Interior of microwave at the prep table in the waitress station has accumulation of grease/food debris. The operator cleaned the interior of the microwave.
  • [31B-03-4] No soap provided at handwash sink. The handsink by the dishwasher area. The operator placed soap.
  • [03F-10-5] No written procedures available for use of time as a public health control to hold time/temperature control for safety food. I provided the operator with the form DBPR Form HR 5022-090.
  • [53B-14-5] Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Amanda H. expired on 4/2026 - the Always Food Safe.
