  • [12A-07-5] Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee discarded gloves and was gonna proceed to put new gloves without wash hands. Coached employee to wash hands.
  • [14-86-1] Non-food grade paper/paper towel used as liner for food container. Observed a pink cloth used inside container with raw shrimp non food grade.
  • [12A-09-4] Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee touch personal phone and proceeded to grab to cut cooked shrimp at sushi station.Coached employee to wash hands properly.
  • [02C-03-5] Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed milk container not dated at server area. Employee dated.
  • [31A-11-4] Handwash sink used for purposes other than handwashing. Observed a container stored inside hand sink by cook line. Observed a drainer stored inside hand sink by sushi station hand sink.
