  • [25-05-4] Single-service articles improperly stored. In back storage room, case of napkins stored on floor. Manager grabbed two milk crates and raised napkins off floor.
  • [36-73-4] Floor, wall and/or ceiling soiled/has accumulation of debris. Wall behind cook line soiled with grease debris.
  • [12A-27-4] Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Employee on cook line cracked raw shelled eggs in pan for customer order and with no handwash just glove change began to handle clean utensils and plates for other orders and cooked toast. Manager coached employee on proper handwashing procedures.
  • [22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior deflector plate of ice machine soiled with black/pink mold-like substance.
  • [24-05-4] Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Ice bucket in front of ice machine not inverted. Manager flipped bucket over.
  • [33-16-4] Open dumpster lid. Right top lid open on dumpster. Manager shut lid.
  • [08B-74-1] Water softener salt (food) stored in a location that is not clean and dry. In dry storage room, bag of water softener pellets stored on floor in front of water tank. Manager removed bag from floor.
  • [12B-02-4] Employee eating in a food preparation or other restricted area. Employee on cook line finished making eggs for customer order and while turning around to put pan in dish employee ate leftover eggs in pan from order.
  • [08B-17-4] Unwashed fruits/vegetables stored with ready-to-eat food. In walk-in cooler, case of unwashed tomatoes stored on shelf above bagged cut onions. Manager moved tomatoes to lower shelf and onions above them.
