  • [06-09-1] Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Inside reach-in cooler: Four packages of fish thawed inside reduced oxygen packaging. The fish is completely thawed. Package does have a label indicating the fish must be removed from packaging prior to thawing. Stop sale issued.
  • [14-09-4] Cutting board has cut marks and is no longer cleanable. Cutting board at refrigerated make-table has deep cut marks and is no longer smooth and cleanable.
  • [08B-38-4] Food stored on floor. Boxes containing tortillas and seasoning stored directly on floor. Manager removed both items from the floor during this inspection.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gasket on door of reach-in freezer soiled with mold-like substance.
  • [50-17-3] Operating with an expired Division of Hotels and Restaurants license. License expired 06/01/2025. MFDV is open and operating during this inspection.
  • [01B-13-4] Stop Sale issued due to food not being in a wholesome, sound condition. Inside reach-in cooler: Four packages of fish thawed inside reduced oxygen packaging. The fish is completely thawed. Package does have a label indicating the fish must be removed from packaging prior to thawing.
  • [31A-09-4] Handwash sink not accessible for employee use at all times. Handwash sink blocked by trash can and large containers of oil. Manager unblocked the sink during this inspection.
  • [31A-11-4] Handwash sink used for purposes other than handwashing. Sanitizer bucket stored down inside handwash sink. Manager removed the bucket from the sink during this inspection.
