  • [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employees purse and bag stored with single-use items below bar. Manager had employees remove all personal items from the shelf during this inspection.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets on keg cooler doors soiled with mold-like substance.
  • [33-16-4] Open dumpster lid. Dumpster lid left open.
  • [25-06-4] Single-service articles not stored inverted or protected from contamination. Several boxes containing single-use cups and lids stored directly on floor in storage room behind bar. Manager placed all items on a table during this inspection.
  • [29-49-6] Standing water in bottom of reach-in-cooler. Standing water inside bottom of keg cooler in bar area.
  • [14-06-4] Wood surface not properly sealed. Unsealed wooden shelves in liquor storage closet in bar area.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Interior of keg cooler in bar area soiled with dirty water and/or black substance.
  • [11-26-1] No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Two new employees working in bar area handling food. Manager has no proof wither employee has been informed of their health reporting responsibilities.
