  • [08B-38-4] Food stored on floor. Containers of frosty mix stored on floor of walk in cooler. Operator stored properly.
  • [22-49-4] Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. As per operator dish machine is out of service. No out of service sign was posted. Operator affixed out of service sign to dish machine.
  • [12A-13-4] Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee handled mop to mop floors then handled clean utensil to stir chili; no hand wash. Employee washed hands and cleaned and sanitized utensil.

Time/temperature control for safety food reheated for hot holding not reaching 165 degrees Fahrenheit for 15 seconds within 2 hours. At steam table: chili with left over burgers(101F at 8:36am; 138F at 9:40am- Reheating) As per label and operator, since 7:39am. Chili did not reheat to 165F within 2 hours. See stop sale.
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At low boy reach in cooler at front counter: half and half (45F - Cold Holding) As per operator, removed from cooler for order 15 mins prior. Not prepped or portioned today. Operator placed to quick chill.
  • [03E-02-5] Time/temperature control for safety food reheated for hot holding not reaching 165 degrees Fahrenheit for 15 seconds within 2 hours.
