  • [50-09-4] Current Hotel and Restaurant license not displayed.
  • [14-09-4] Cutting board has cut marks and is no longer cleanable. Observed grooved cutting boards on the cooks line across from the stove and in the ware washing drying rack.
  • [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee phone on front counter used to place to go orders,
  • [14-11-5] Equipment in poor repair. Dish machine not properly working, operator using 3 compartment sink for ware-washing.
  • [35B-08-4] Fly sticky tape hanging over food/food preparation area/food-contact equipment, close to food service window, operator removed fly paper from area and washed his hands.
  • [08B-38-4] Food stored on floor. Observed food stored on floor in the walk in freezer and dry storage, operator raised items off the floor to appropriate shelf.
  • [24-17-4] Silverware/utensils dried with a towel/cloth. Observed dishwasher drying bowls and utensils with cloth, operator asked employee to let the items air dry.
  • [08B-17-4] Unwashed fruits/vegetables stored with ready-to-eat food. Observed unwashed produce in walk in cooler stored over ready to eat broth, operator said he will moved the items to correct location.
  • [42-01-4] Wet mop not stored in a manner to allow the mop to dry in the back kitchen , operator hung mops to dry .
  • [10-05-5] In-use utensil stored in unclean water at or above 135 degrees Fahrenheit, observed flat spoon stored in water used to dispense rice in water at 81f, employee discarded the water container.
  • [35A-02-7] Live, small flying insects found Observed 3 flying insects in the kitchen area
  • [01B-02-5] Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed cooked chicken nuggets at 71f cooked the day before at 8 pm ,operator discarded item.
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed cooked chicken nuggets at 71f above the cool line in the reach in cooler ,operator states nuggets cooked the day before , operator discarded item.stop sale issued Raw chicken in walk in cooler temped at 44f operator place chicken in walk in freezer to quick chill.
  • [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet, in back kitchen.
  • [41-27-4] Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed Quat sanitizer measured over 500 ppm , operator corrected sanitizer ppm in spray bottle .
  • [31A-11-4] Handwash sink used for purposes other than handwashing. Observed employee tensing rice noodle in the handwashing sink next to stovetop area, employee moved item to prep sink.
  • [02C-02-5] Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked.did not observe dates on any items prepared more than 24 hours.
