- [32-07-5] During the callback inspection the bathrooms were cleaned in some area but they still need to be deep cleaned and also had a very unpleasant smell.
- [36-37-5] Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas.
- [36-32-5] Ceiling/ceiling tile shows damage or is in disrepair. Observed multiple ceiling with water damage throughout the establishment.
- [36-34-5] Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance.
- [14-11-5] Sliding reach in cooler empty with food of any kind, operator stated that theyre waiting for the technician.

- [03B-01-6] During the callback inspection observed cheese pizza 108F, shrimp 100F, fried chicken wings 90F. As per operator food was put out in the buffet line about 1 hour ago.
