  • [02B-04-5] Menu contains an item made with a raw/undercooked animal food as an ingredient and description of item does not inform consumer of raw/undercooked animal food ingredient. Bar menu missing disclosure for caviar, tuna stack, regular menu and bar menu missing disclosure for garlic aioli. Also, bar menu missing consumer advisory.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Make line cutting boards with staining in main and pantry kitchens.
  • [25-18-4] Single-service articles stored in toilet room/locker room/garbage room/mechanical room. Observed single service items stored in employee restroom. Person in charge began removing.
  • [36-17-5] Floor tiles missing and/or in disrepair. Multiple tiles cracked in kitchen area.
  • [35B-01-4] Exterior door has a gap at the threshold that opens to the outside. Light visible under back door.
  • [41-17-4] Spray bottle containing toxic substance not labeled. One spray bottle unlabeled with cleaner in main kitchen. Person in charge labeled appropriately.
  • [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink. Paper towels missing at hand wash sink at bar.
  • [51-16-7] No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. The direct link to the Plan Review page is https://www2.myfloridalicense.com/hotels-restaurants/licensing/plan-review/. Establishment is utilizing server area to fill pitchers from ice buckets. Ice bin and hand wash sink on original plans have been removed. Also, establishment has added glass cleaner to dishpit.
  • [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. Raw shell eggs stored over butter in walk in cooler. Also, créme brûlée mix with raw pooled shell eggs stored over ready to eat items. Also, raw ground beef stored over whole intact steak in walk in cooler.
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Blue cheese stuffed olives on bar at 60F. Recommend holding on time.
  • [01B-13-4] Stop Sale issued due to food not being in a wholesome, sound condition. Frozen fish (tuna and snapper) vacuum sealed in house, fish is thawed when sealed per person in charge. Also, one package of in house vacuum sealed salmon thawed in walk in cooler.
