  • [08B-38-4] Food stored on floor. Observed a case of onions stored on the floor at prep area.
  • [06-09-2] Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. See stop sale. Observed a whole fish thawed in original ROP packaging at walk-in cooler.
  • [24-05-4] Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed at storage area pots, and pans, not inverted.
  • [36-02-5] Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Observed throughout kitchen and prep area floor not sealed.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed under table, vegetable, dicer, soiled with old food manager, had item sent to dishwasher to clean.
  • [01B-13-4] Stop Sale issued due to food not being in a wholesome, sound condition. Observed a whole fish thawed in original ROP packaging at walk-in cooler.
  • [53A-01-7] Manager or person in charge lacking proof of food manager certification. Observed Laura with no proof of manager certification
  • [29-44-4] Air gap between water supply inlet and flood rim of equipment/sink is less than one inch. Observed at 3 compartment sink.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gasket at reach in cooler soiled with mold, like substance manager cleaned during inspection.
  • [51-16-7] No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days.   • [31A-04-4] Handwash sink removed from food preparation/dishwashing area. Must be reinstalled in the same location where removed. Observed HWS removed at end of cook line.
