  • [02C-02-5] Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed meatballs with no date marking. As per operator, cooked day before. Date was placed.
  • [11-26-1] No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed Alex, Violeta, Emmanuel working for less than 60 days. Provided form to operator via email.
  • [14-09-4] Cutting board has cut marks and is no longer cleanable. Observed white cutting boards grooved at front counter area.
  • [05-09-4] No conspicuously located ambient air temperature thermometer in holding unit. Observed at salad RIC at front counter.
  • [29-49-6] Standing water in bottom of reach-in-cooler. Observed at RIC next to ice machine.
  • [53B-16-4] Employee has not received adequate training related to their assigned duties as evidenced by the inability to answer basic food safety questions. Observed staff not knowing proper quaternary concentration at 3CS. Coached staff on correct concentration.
  • [45-04-4] Use of cooking equipment producing grease laden vapors/smoke with no hood suppression system installed. Notified Fire AHJ. For reporting purposes only. Observed electric flat grill and electric range with two burners being used to cook at preparation area next to oven.
  • [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw sirloin stored over cooked meatballs at RIC at preparation area. Operator moved products accordingly.
  • [50-09-4] Current Hotel and Restaurant license not displayed. Observed license posted from October 1, 2025.
  • [31B-04-4] No handwashing sign provided at a hand sink used by food employees. Observed at only bathroom.
  • [33-16-4] Open dumpster lid.
