  • [10-20-4] In-use tongs stored on equipment door handle between uses. Observed two tongs on oven handle at cook line.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed waffle iron soiled with debris. Observed can opener blade at prep table soiled with food debris.
  • [24-05-4] Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed plastic containers at side storage, not inverted.
  • [31A-11-4] Handwash sink used for purposes other than handwashing. Observed in handwashing sink at cook line a metal bowl and food debris.
  • [42-01-4] Wet mop not stored in a manner to allow the mop to dry. Observed mop at mop sink not hung to dry.

  • [16-21-4] Accumulation of debris on exterior of warewashing machine. Observed exterior of dish machine soiled with debris.
  • [14-09-4] Cutting board has cut marks and is no longer cleanable. Observed three shot cutting boards at cook line with deep marks and not easily cleaned.

  • [35A-03-4] Dead roaches on premises. Observed two dead roaches on the floor at warewashing area. Employee swept and discarded dead roaches.
