  • [14-11-5] Equipment in poor repair. Gaskets in poor repair at left side cook line flip top cooler
  • [08B-38-4] Food stored on floor. Bucket of sprouts stored on floor in walk in cooler. Operator moved to shelf
  • [14-13-4] Food-contact surface not smooth and easily cleanable. Cut off container used as rice scoop. Operator discarded.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Exterior of microwave on. Pork line soiled
  • [22-16-4] Reach-in cooler interior/shelves have accumulation of soil residues. Observed at left side cook line flip top
  • [12A-07-5] Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee put on gloves and begin handling containers of food without first washing hands. Discussed with operator operator instructed employee to remove gloves and wash hands
  • [14-15-4] Nonfood-grade containers used for food storage - direct contact with food. Observed cut cabbage stored in direct contact in cardboard box in walk in cooler. Operator moved to food grade container
