[23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler gaskets with mold like substance at the sushi station.
[14-11-5] Equipment gaskets in poor repair. Observed gaskets in disrepair at the sushi station .

To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed expired food handler training for : Juan Cuevas, Victoria Senff, Erik Marcel, Geston Sylvain, Ivan Martinez.

[10-20-4] In-use tongs stored on equipment door handle between uses. Observed tongs were hanging on shelf exposed to cross contamination. Employee moved and washed them immediately .

[31A-07-4] Handwash sink missing in warewashing or food preparation area. Observed employee dining area with missing hand sink. Food is served at the location .
[51-14-7] No plan review submitted and approved - buffet area / employee dining area and new food area were added. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Observed food area was added at the first floor bar by men locker room, employee dining area and buffet on the second floor dining area.
