  • [22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed residue buildup in the interior of ice machine. Operator cleaned and sanitized the ice machine.
  • [36-36-4] Ceiling tile missing. Observed removed tile from ceiling at back interior entrance to the kitchen. Operator placed tile back in the ceiling.
  • [36-34-5] Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed soiled ceiling tiles/vents at various locations at kitchen.
  • [14-09-4] Cutting board has cut marks and is no longer cleanable. Observed grooved cutting board at Make Table.
  • [10-12-5] In-use ice scoop stored on soiled surface between uses. Observed ice scoop on exterior soiled shelf of ice machine. Operator cleaned and sanitized the ice scoop and placed it in a clean container.
  • [10-01-5] In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed scoop handle touching sugar at container by wait station. Operator removed the handle from touching the container.
  • [02D-01-5] Working containers of food removed from original container not identified by common name.
