  • [10-06-5] In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Observed handle touching the rice on the steam table at the cook line. Manager removed the handle.
  • [14-11-5] Equipment in poor repair. Observed reach in cooler door gaskets torn at the cook line.
  • [35B-01-4] Exterior door has a gap at the threshold that opens to the outside.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler door gaskets soiled at the cook line. Also observed hood filters soiled.
  • [03B-01-6] Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed beef (87 F - Hot Holding)on top of the stove. As per employee food was placed around 11:30 AM. Employee reheated food to 165°F. Also observed ribs (130 F - Hot Holding) inside the steamer in the kitchen. Employee will reheat the ribs.
  • [06-09-1] Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed 2 salmons no longer frozen and not removed from reduced oxygen package. Stop sale was issued.
  • [25-06-4] Single-service articles not stored inverted or protected from contamination. Observed spoon stored not inverted at the front line. Employee stored them correctly.
  • [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed splitter added to hose Bibb. Hose was connected at the time of the inspection.
  • [01B-13-4] Stop Sale issued due to food not being in a wholesome, sound condition. Observed 2 salmons no longer frozen and not removed from reduced oxygen package. Stop sale was issued.
  • [08B-12-5] Stored food not covered. Observed fish and chicken not covered inside the walk in freezer. Employee covered the food.
  • [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink. Observed at the hand wash sink by the fryer. Employee provided paper towels.
  • [36-34-5] Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed vents soiled all around dining area.
