  • [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee purse on shelf labeled "employee" is stored over take out boxes in downstairs bar.
  • [01B-13-4] Stop Sale issued due to food not being in a wholesome, sound condition. Four packages of completely thawed and vacuum sealed mahi in reach-in cooler across from fryer.
  • [21-08-4] Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Wiping cloth solution at triple sink tested at 0ppm quaternary ammonium. Operator found chemical empty, replaced chemical and retested at 150ppm quaternary ammonium.
  • [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Vacuum breaker missing on threaded faucet with attached hose outside back kitchen exit. Operator located vacuum breaker and installed during inspection.
