[14-11-5] Equipment in poor repair. Torn door gasket to stand up reach in freezer.
[22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. At front counter, blender machine with accumulation of spilled food debris on base and wall of blender. Operator asked employee to clean.
[16-10-4] Abrasives and detergents not removed during the rinse process. On shelf opposite three compartment sink, clean containers stacked while wet with soap visible on edges of containers.
[35A-02-7] Live, small flying insects found. Observed one live fly around front counter cook line area, operator dismissed fly to lobby area.
[41-15-5] Wiping cloth solution exceeding the maximum concentration allowed stored in a location that could result in the cross contamination of food, equipment, utensils, linens, single-service, or single-use articles. Two sanitizer solutions, one at front counter and one on cook line at 200+ ppm turned strip black. Operator had employee empty and reset sanitizer solutions to 100ppm.
[23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. At front counter coffee station, sliding holder for plastic cups with debris build up in circular holder. Operator asked employee to clean and sanitize. Hood filters over fryers have grease build up on filters.
