[40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employees phone stored in pan directly on top of syrup packets on shelf in front of kitchen. Manager removed the phone from the area during this inspection.
[13-07-4] Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Several employees handling food in kitchen with bracelets and/or watches on their wrists.
[23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. 1. Area around nozzles of fountain machine in lobby soiled with mold-like substance. 2. Interior of cabinet below fountain machine in lobby and wrap around drain lines inside cabinet soiled with mold-like substance.
[21-07-4] Wiping cloth chlorine sanitizing solution not at proper minimum strength. Sanitizer bucket chlorine 0ppm. Manager had an employee dump and refill the bucket, then chlorine 50ppm.
[14-16-4] Multiuse food-contact surface not smooth, free of breaks/cracks/chips/pits/crevices. Top of opening cracked near hinge of ice machine lid.
[16-37-1] No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No chlorine test kit available for chlorine sanitizer in sanitizer buckets.
[31B-06-4] Soap dispenser at handwash sink not working/unable to dispense soap. 1. Soap dispenser at handwash sink at cookline missing faceplate and unable to dispense soap. 2. Soap dispenser at handwash sink near drive through unable to dispense soap. The manager stated the dispenser may need a new battery and placed a hand pump soap bottle at the handwash sink during this inspection.

[23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Area around nozzles at fountain machine in seating area. Manager had employee begin cleaning fountain machine during this inspection.
[12B-13-4] Opened employee beverage container in a cold holding unit with food to be served to customers. Two employee drinks on shelf above fish inside reach-in freezer at cookline. Manager removed drinks from freezer during this inspection.
[08B-12-5] Stored food not covered. Canister of tea uncovered near drive-through window. Manager placed lid over tea during this inspection.
[36-50-4] Unclean building components, attachments or fixtures. Floor in outdoor dry storage shed soiled with dirt.
[12A-28-4] Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee on cookline touched pants and apron with gloved hand and continued operating the fryer. Manager had employee stop, wash hands, and put in new gloves before continuing to operate fryer.
[03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In pan behind front counter: butter packets (64F - Cold Holding). Manager stated the butter had been out of refrigeration for approximately one hour and placed butter packets in walk-in freezer, where they cooled to 43F.
[41-10-4] Toxic substance/chemical improperly stored. Spray bottle containing multi-surface cleaner stored on shelf above boxed sodas near drive-through window. Manager removed spray bottle from shelf during this inspection.
[13-06-4] Employee with artificial nails/nail polish working with exposed food without wearing intact gloves. One employee with artificial nails prepared drink at pickup window without wearing gloves. Manager had employee put on gloves before continuing to work.
[22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Nozzle at fountain machine in seating area heavily soiled with mold-like substance. Manager had employees begin cleaning nozzles during this inspection.
