- [36-73-4] Floor soiled/has accumulation of debris. Floors in walk-in cooler and under three compartment sink are soiled.
- [22-16-4] Reach-in cooler/Walk-in cooler interior/shelves have accumulation of soil residues. Walk-in cooler shelving has build up of soil residues.
- [08B-12-5] Stored food not covered. Cut cabbage, broccoli, wontons and chicken stored uncovered in walk-in cooler.
