  • [14-69-4] Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup in up in the reach in freezer in the kitchen.

  • [01B-04-5] Stop Sale issued due to food originating from an unapproved source. Observed two carton of hard shell eggs sitting out on the counter out front for sale, owner stated server brings them from home and sells them, this is from unapproved source. See stop sale
  • [13-07-4] Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed bracelet on the cook while preparing food, cook took bracelet off.
  • [16-55-4] Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Observed dish machine the chlorine solution read 00ppm, owner changed out the solution it read at 50 ppm.
  • [24-08-4] Equipment and utensils not properly air-dried - wet nesting. Observed stack of cups in the wait staff area with water in between and not air drying properly.
  • [08B-01-4] Self-service salad bar/buffet lacking adequate sneezeguards or other proper protection from contamination. Observed sneeze guard was up on the salad bar, owner put down.
  • [27-06-4] No hot running water at three-compartment sink. Observed no hot water at the 3-compartment sink in the bar area.
  • [33-16-4] Open dumpster lid. Owner closed.
  • [08B-02-4] Displayed food not properly protected from contamination. Observed a container of croutons not protected at the salad bar, owner moved croutons under the sneeze guard.
  • [14-11-5] Equipment in poor repair. Observed torn gasket in the reach in cooler in the kitchen.
  • [45-02-4] Portable fire extinguisher gauge in red zone. For reporting purposes only. Observed fire extinguisher in the red in the bar area.
  • [14-01-5] Bowl or other container with no handle used to dispense food. Observed bowl in container of rice, owner took out the bowl.
  • [22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed a green like substance on the white guard in the ice machine.
