  • [31B-04-4] No handwashing sign provided at a hand sink used by food employees. No employee hand wash signage at hand wash sink by bulk ice bin.

Instructed to keep lid closed as much as possible or relocate to interior of cooler.

Sides of cooking equipment on cook line with grease and food debris. Top of pizza oven with accumulation of dust. Food debris in reach in cooler door gaskets. Dust build up on walk in cooler compressor fans. In walk in cooler, beer kegs with mold like substance on the exterior and on lines.
  • [35A-02-7] Live, small flying insects found. Observed two live flies, one in prep area by table top warmer and on cook line between pizza oven and reach in cooler.
  • [10-20-4] In-use tongs stored on equipment door handle between uses. Tongs stored on pizza oven door handles.
  • [08B-12-5] Stored food not covered. In standup reach in cooler, soup, chicken, beef, fish, shrimp all uncovered. Jugs of fryer oil stored on the floor.
  • [36-73-4] Floor, walls and ceiling soiled/has accumulation of debris. Floors under cook line and prep area soiled with debris and grease. Ceiling over cook line with dust build up around ceiling light. Mold like substance on wall behind three compartment sink. Floor under shelf in walk in cooler at end of cooler soiled spilled food debris.
