- [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed : chicken cooked earlier at 93°, sitting on the drain board. This was moved to the walk-in cooler, partially covered.
- [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed : a jacket and cell phone stored over and next to bulk food on the dry goods shelf these personal items were moved to a chair and below.
- [51-13-4] No Heimlich maneuver/choking sign posted. Observed : lack of a visible choking poster. One was emailed.
- [31B-04-4] No handwashing sign provided at a hand sink used by food employees. Observed : lack of a sign at the front line hand wash sink. Copies were emailed.
- [02D-01-5] Working containers of food removed from original container not identified by common name. Observed : bulk sugar and seasonings are not clearly labeled. Please add Chines and English labels.
- [41-10-4] Toxic substance/chemical improperly stored. Observed : spray WD-40 stored over bulk sugar, in dry storage.
- [23-24-4] Buildup of food debris/soil residue on equipment door handles. Observed : the walk-in cooler door handle has an accumulation of food debris on the backside of the handle.
- [08A-17-6] Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Observed : in the back reach-in freezer, raw chicken over repacked beef. ALSO, on the cook line, the chicken was on the back row in the make table, followed by shrimp and beef. The new order was chicken-beef and then shrimp.

- [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed : the hood filters over the wok station have an accumulation of grease.
- [08B-49-4] Employee personal food not properly identified and segregated from food to be served to the public. Observed : 3 bags of frozen food on the drain board of the sink. These were then marKed emp and moved below.
