- [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Soda gun holsters soiled with a slime like substance.
- [24-14-4] Clean utensils stored between equipment and wall. In use utensils stored between prep tables ready for use. Employee moved utensils to dish machine to be properly cleaned.
- [27-16-4] Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Bathroom on left side of restaurants sink did not have any hot water when turned on.

- [14-01-5] Bowl or other container with no handle used to dispense food. Small sample cup used to scoop cottage cheese found left in container. Employee removed container and threw it away to correct on site.
- [24-08-4] Equipment and utensils not properly air-dried - wet nesting. Clean containers stored on shelf before properly drying near dish machine.
- [12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employ drink container held on hot holding soup table.
- [36-73-4] Floor soiled/has accumulation of debris. Floor of walk in cooler had build-up of debris and was soiled with a slime like substance.
- [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Mixer head on stand mixer had a buildup-up of residue underneath where mixer blades are attached. Ice sticks stored on ground in walk in freezer.

Dishwasher was seen handling dirty dishes and then working with clean dishes without proper handwashing. Discussed with operator on the importance of issues and ways to negate the situation.
