  • Disposition: Warning Issued
  • [21-07-4] Wiping cloth chlorine sanitizing solution not at proper minimum strength. Observed Sanitizer Bucket (Chlorine 0ppm) near triple sink. Operator remade solution and retested at 50ppm.
  • [53A-02-7] Manager or person in charge lacking proof of food manager certification and no other certified food service manager employed at this location. Observed operator unable to provide proof of CFM during this inspection. Operator stated he grabbed the wrong folder that has the paperwork.
  • [08B-38-4] Food stored on floor. Observed container of oil on floor. Operator removed oil to properly store.
  • [50-04-4] Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. Observed MFDV numbers not affixed to the exterior of food truck.
  • [11-26-1] No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Operator unable to provide proof that employees are informed of reporting symptoms related to food borne illnesses to person in charge. Distributed health form to operator during this inspection.
  • [09-01-4] Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Employee touched cooked ready to eat burger that was not going to be reheated to 145F with bare hands and no AOP. Discussed with operator. Employee washed hands.
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed sliced American cheese, sliced pepper Jack cheese (46F - Cold Holding) in top of flip top reach-in cooler. Lid left open. Operator closed lid to chill cheeses.
  • [41-10-4] Toxic substance/chemical improperly stored. Observed spray bottle containing toxic substance stored on top of reach-in cooler. Operator removed spray bottle of sanitizer to properly store.
