Manager had employee rearrange the shelves for proper storage during inspection. In reach in cooler- Raw beef and pork stored in top portion of make table cooler above cheeses and vegetables with no complete physical barrier between top and bottom.   • [10-07-4] In-use utensil stored in standing water less than 135 degrees Fahrenheit. Spoon in 96F water in cook line area. Operator removed to be washed.
  • [21-12-4] Wet wiping cloth not stored in sanitizing solution between uses. In cook line area on prep table. Operator placed wiping cloth into a sanitizer bucket.
  • [23-03-4] Exterior area of ice machine soiled above lid. Operator had employee clean and sanitize the area during inspection.
  • [32-12-6] Covered waste receptacle not provided in womens bathroom.
  • [05-09-4] No conspicuously located ambient air temperature thermometer in holding unit. Operator placed ambient thermometer in the reach in cooler during inspection.
  • [41-17-4] Spray bottle containing clear toxic substance not labeled. Operator discarded the chemical inside the bottle.
  • [22-08-4] Interior of microwave has accumulation of food debris in cook line area. Operator had employee clean during the inspection.
  • [11-26-1] No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. No proof for any employees. Provided operator with employee reporting agreement.
  • [12A-28-4] Employee in cook line area. touched soiled apron, then handled clean utensil without removing gloves and washing hands. Discussed with manager.
  • [02B-02-5] Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. Provided advisory to the manager who printed and posted during the inspection.
  • [29-03-4] Water draining onto floor surface. Ice machine draining directly onto floor, pipe has been disconnected. Operator connected pipe during inspection.
  • [31A-11-4] Handwash sink used for purposes other than handwashing. Wiping cloth stored in hand wash sink in server station. Operator removed cloths.
